      • Name changes: If the current names of either parent are different to how they appear on the child’s birth certificate, please include certified documents that explain the change of name (for eg. Marriage certificate, B11 General Declaration explaining the difference in their name).
  • Please ignore section 16 Page 85F for child/ minor applications
  • Print the application form
    • Print the application on plain white A4 paper, using black ink. Make sure the form has been printed in full and isn't cut off at the top or bottom of the page.
    • US standard letter paper or US legal paper sizes are also acceptable, but only if they’re scaled properly. Refer to more information on printing your application form.
  • Sign the application form
    • Sign the form using black pen, within the signature box. Make sure there are no alterations or whiteout on the signature or the date sections. Both the parents signing on section 15 Page 84F is declaration and consent to the child applying for an Australian Passport.
    • Children 10 years or older must provide a signature (section 18 Page 85F) unless there is a medical reason or disability that prevents them from signing.
    • Please do not enclose any original documents or the applicants current Australian Passport
  • Arrange a guarantor signature for section 11 Page 83F of the application form.
  • Arrange for both parents providing their consent to sign section 15 of the form in front of a witness. The witness should not be related in any manner to either parents or the applicant. Both parent’s consent is mandatory for a child/ minor passport application.
  • Three or more identical colour photos (glossy finish only), less than 6 months old with their full name written (in capital letters) and endorsed by the guarantor on the reverse of one photograph. Refer to photo guidelines for more information.
    • Do NOT attach the photos to the application form as this may damage them. We suggest sealing the photo (photo with the applicant’s name written) in a separate sachet so that the ink does not smear on the other photos thereby causing damage. Do NOT use jump clips or staple pins on the application form or photographs. This will cause damage and probably result in longer turnaround times in processing of your passport application.
    • Request your photographer to provide you with a soft copy of the applicant’s photograph in case there are any issues with the print quality.
  • Include a self-addressed envelope with your Indian mobile number. You must ensure the address is complete with landmarks if any and the post-code. This will enable the courier company to delivery your renewed Australian Passport in a prompt manner.
  • Post or courier (you may use any company that suits your convenience) your application to the Australian Consulate-General, Chennai.
